Mangal Dosha is present when Mars occupies the 1st, 2nd, 4th, 7th, 8th or 12th house, counted from the Lagna or from the Moon. Both reference points are checked, because a chart can be clean from one and afflicted from the other. The result names the house, so you can see the reasoning rather than a bare yes or no.
Six of twelve houses count, and the count is taken from two different reference points — so a large share of charts qualify under the strict rule. That is a property of the rule, not a sign that something is unusually wrong with your chart. Classical practice reads the strength and the dignity of Mars, and the rest of the chart, before treating it as a serious matter.
Traditional texts list a number of conditions under which the dosha is considered cancelled or reduced — among them both partners carrying it, Mars in its own or exalted sign, and certain aspects from benefics. This is the point at which a real astrologer is worth more than a calculator.
